By purchasing this listing, you agree to send your 2004 Axiom DRX9000 Lumbar Computer for Repair Estimate. This is not a new or replacement of a 2004 Axiom DRX9000 Lumbar Computer.
WHERE TO SEND THE COMPUTER FOR REPAIR ESTIMATE?

IMPORTANT:
- The computer should be packaged in a box. There should be a minimum of 3 inches of space around the unit for extra padding (bubble wrap or foam). It is very important to put extra padding when you ship heavy items.
- Remember to get the tracking number for your shipment. You have the option to insure your DRX9000 Computer. Please make inquiries with your carrier for rates. You will also have the option to insure your DRX9000 Computer on its way back to you (out-of-warranty service only). The insurance rates will be provided in the repair estimate sent to you once the unit has been evaluated.
- Freight claim responsibility and insurance liability responsibility both belong to the customer
- We will contact you within 2-3 business days from the time we receive your package with the estimate and time needed for repair. We will not begin any work on your equipment without your approval.
- We accept Visa, Master Card, Discover, and American Express. We also work with Purchase Orders.
REPAIR POLICY:
THE COMPUTER MUST BE IN GOOD PHYSICAL CONDITION WITHOUT ANY VISUAL DAMAGE AND WITHOUT ANY COMPONENTS ON THE COMPUTER MISSING OR BEING ALTERED.
In most cases, we can repair the defective computer, power, or panel screen. If your board can’t be repaired, we will contact you and do the refund (no repairing fee, pay return shipping fee only)
All items we repair are guaranteed to be complete with a 100% working order and not DOA. Warranty will be voided for any parts damaged due to improper installation, setup, or any other human error. For security, all boards are marked with unique company markings. All defect/warranty claims will be inspected for our company markings, any parts returned missing these markings will not be accepted and you will be notified.
